Near North Health Services Corporation
Near North Health Service Corporation (Near North), our community health partner for more than 40 years, provides comprehensive primary care, social services and dental services to primarily low-income uninsured or underinsured residents who live in some of the city’s most medically underserved communities.
To address increasing community needs, Near North expanded to provide clinical and social services through eight health centers, making it one of the largest providers of community-based primary care in Chicago. Erie Family Health Center
Erie Family Health Center (Erie) was founded in 1957 as a project of volunteer physicians from Northwestern Memorial and Erie Neighborhood House. Erie provides a variety of primary care, case management and dental services to over 37,500 low-income, underinsured and uninsured patients per year.

CommunityHealth is the largest, all-volunteer free clinic in Illinois. For 20 years, CommunityHealth has been dedicated to providing comprehensive, high quality, patient-centered health care at no cost to low-income, uninsured adults in need of a medical home. In 2012, CommunityHealth provided more than 25,500 medical and dental visits for over 10,000 patients.
